To meet the unique needs of our patients, Autumn Woods Health Care Facility features state-of-the-art unit designed specifically for full-scale subacute and orthopedic rehabilitation and recovery. Our experience in the subacute arena spans across two decades, with an average of 1,000 patients a year who trust us to manage their rehabilitation and healing.
There are many things to consider when choosing a rehabilitation facility. We recommend taking a tour of the subacute unit, meeting the therapy staff and speaking with your doctor about the type of program that’s best for you.
The Encore rehabilitation programs focus on the treatment of the following:
- Chronic Conditions
- Joint Fractures
- Neurological Conditions
- Neck and Back Injuries
- Post-Operative Conditions
In addition, our therapists possess advanced degrees, specializations and certifications that provide a broad and comprehensive knowledge base to treat many injury, pains or therapy-related concerns.
As a patient, you will receive therapy on a dedicated unit, enjoy restaurant-style dining, utilize top-of-the-line therapy equipment and train in ADL functions, which provide opportunities to practice in a simulated home environment.
If you’re considering elective surgery in the near future, it’s wise to begin researching your rehabilitation options now. Pre-planning your stay will allow you to seamlessly transition from the hospital to our facility and immediately begin the healing process.
Remember, you have a right to choose your rehab facility. As our patient, your rehab experience will be second to none; because helping you heal and getting you back home is more than our desire at Autumn Woods, it’s our mission.
