Our mission statement is “Through our care, we will positively affect the lives of our residents, staff, family members and the community at large.”
For more than 30 years, our staff has touched the lives of more than 20,000 patients. We provide the same care to all of our patients, regardless of payment source – every resident receives the same respect and compassion.
The facility does not segregate residents based on payment source and is fully certified in all beds for Medicaid, assuring that everyone receives the same high quality health care experience. Our Medicaid beds are not limited, we do not segregate private, Medicare or Medicaid patients to a particular area, and payment sources are not a deciding factor in admission or placement. Autumn Woods participates in all Medicaid Programs and does not discriminate based on payment source.
Our job is to meet the needs of the community at large – we exist to meet their needs.
Autumn Woods is the only VA certified facility in Macomb County.
Organizational Description
Autumn woods is a single owner, 293 bed stand-alone facility under the same ownership since opening in 1982.
We provide sub-acute rehab services, Home Hemo-Dialysis on site, skilled long-term care services, dementia care with a secured unit, hospice and respite, and palliative care as well as long-term nursing services.
Our size and independence allows us to adapt quickly to market changes, implementing new programs and policies that reflect the needs of our patients, staff, and referring institutions. Our non-union staff is stable, long-term and highly valued by the management who have an average tenure of over 20 years.
Descriptive Information
Environment – Autumn Woods is a three story structure on 4.5 acres in Warren, Michigan located 0.5 miles north of 12 mile road.
The 293 beds are all private and semi-private fully air conditioned rooms, in a home-like setting. All of our housekeeping, laundry, dietary, maintenance and nursing services are provided by Autumn Wood’s staff.
Subcontracted Services
he facility has contracts for therapy, pharmacy, laboratory, radiology, respiratory, hospice, and transportation.
All providers are in good standing with CMS for Medicare and Medicaid Programs and are licensed as appropriate within the State of Michigan.
Clinical Specialties Provided – Therapy is provided 7days/wk; complex medical patients have the advantage of an on-site consultant nephrologist, cardiologist, pulmonary specialist, wound physician, infectious disease specialist, physiatrist, and psychiatrist. These specialist manage the patients in-house in an effort to prevent recurring hospitalizations.
IV therapy is done on a regular basis and the consultant physicians assist with medication management, again to prevent re-hospitalization.
Our safe, secured unit incorporates the latest research and design enhancement to meet the needs of dementia patients while our structured programs keep them active and engaged. Our team focuses on providing emotional and therapeutic care to maintain the existing strengths and abilities of each person.
The facility has 10 bariatric beds and additional required equipment to meet the special needs of these individuals.
Clinical Care Expectations
Our facility offers 7 day/wk, 24 hour/day admissions. Our open medical staff model allows all attending physicians the opportunity to follow their patients into the facility to maintain consistent care and prevent re-hospitalization.
If they choose not to, one of our facility physicians will follow them until discharge, when they are turned back to the primary care provider.
Physician Coverage
Staffing Model – The Medical Director, Robert Orzechowski, is a physician who is on staff and is employed by St. John Macomb Hospital. He has working relationships with all of our physicians. Our open staff model encourages any physicians who choose to follow their patients into the long-term care setting.
Clinical Care Model – Each patient’s individualized plan of care is developed through interdisciplinary assessment and includes the physician, nursing, dietary, therapy and social services. The staff routinely consults with the attending physicians to assure appropriate care and treatment in an effort to meet the established goals and objectives as stated in the plan of care. The facility requires active participation by each physician who must be available 24/hrs/7d/wk. Many patients’ attending physician is their primary care provider. In the event this is not the case, the attending physician follows the patient through the admission and then the facility provides the primary care provider with a complete discharge summary that is delivered to the office in person to assure continuity of care .
Payment
Our facility offers 7 day/wk, 24 hour/day admissions. Our open medical staff model allows all attending physicians the opportunity to follow their patients into the facility to maintain consistent care and prevent re-hospitalization.
